I believe your Driver is what's getting you into trouble. Your irons are strong, so you must have good timing with them. Sometimes, I tend to change the angle of my hands at address with my driver. But the angle at impact or at address should be the same as it is with your irons. You're timing seems to be off with your driver, and the only thing I can think of is that you change your approach/address of your driver swing as opposed to your irons. Try moving the ball further up in your stance, tee it higher, and press your hands well forward ahead of the ball with your driver.
